Established in 2016, Harborside Hospice, LLC is committed to being here for you and your caregiver.   We strive to provide support in all your care needs, whether physical, spiritual or social.

Executive Director

Timothy G. Barry

Timothy G. Barry co-founded Harborside Hospice, LLC in 2016.  Prior to Harborside Hospice, he served as Executive Director/Shareholder at Fidelis Hospice, Director of Business Development at Brookhaven Hospice and Vice President/Director of Business Development at Allegiance Hospice.

Previously, he worked in the Nursing Home industry for approximately twenty-five years.  He was the Regional Director for American Health Centers, Inc. for approximately twenty years, Long Term Care Project Manager for the First Bank of St. Paul in Minnesota and a Nursing Home Administrator for NewMedico, Inc.

He also co-founded three Adult Day Health Centers and Central Wheelchair and Van Transportation, Inc. in Massachusetts.  These businesses have been serving the Merrimack Valley community for over twenty-five years.

Mr. Barry holds a BA from the College of the Holy Cross, MS in Human Service Administration from New Hampshire College and an Advanced Specialization Degree in Educational Administration from Boston College.

Director of Clinical Services

Sylvie J. Hornor

Sylvie J. Hornor co-founded Harborside Hospice, LLC in 2016.  Prior to Harborside Hospice, she served as Sr. Vice President of Clinical Services at Fidelis Hospice, Director of Clinical Operations at Brookhaven Hospice and Administrator/Director of Clinical Services for Allegiance/SolAmor Hospice.

Previously, she worked in the Nursing Home industry for over ten years and held various clinical and clinical management positions.  Prior to entering the hospice industry she functioned as the Director of Nursing Services for Harborside Healthcare.

Mrs. Hornor earned her MSN at Western Governor's University and has gained a wealth of medical knowledge and management experience from navigating the Federal, State and Local Laws and Regulations in Massachusetts, New Hampshire and Maine.

James D. Tollman, MD

James D. Tollman, MD, Fellow in Hospital Medicine, Certified Medical Director, is the founder and President of Essex Inpatient Physicians (EIP). Established in 2007, EIP is a Boxford-based hospitalist group with a primary focus in many post-acute care settings, including LTAC, IRF, SNF, TCH, hospice, VNA, acute inpatient detox hospitals, inpatient Geri-psych and psych-medical care, SNF based duel diagnosis programs and primary care in Assisted Living sites.

Prior to creating EIP, Dr. Tollman was the founding hospitalist of the Lawrence General Hospital group, and its first Medical Director. While at Lawrence General Hospital, he served on the JCAHO CHF, Core Measures, Medical Executive, Medical Quality Review, Family Practice Quality Improvement, Health Information and Code 99 Committees. In 2005, Dr. Tollman became the managing partner of Arbor Inpatient Physicians, a single specialty group that provided hospitalist coverage for six hospitals in MA and CT, covering 980 total beds with 28 physicians.

Before working as a hospitalist, Dr. Tollman was a primary care physician with Lahey Clinic. He attended the Tufts University Family Practice Residency program, is a Fellow in Hospital Medicine, a Certified Medical Director, and graduated from the University of Nebraska College of Medicine. He completed his undergraduate BA in biology from Carleton College in Minnesota and served as an active duty Infantry officer in the US Army for 5 years and in the National Guard Medical Corps for 4 years.

Associate Medical Director
Massachusetts & New Hampshire

Dr. David . Tsai

 Dr. David H. Tsai serves as the Associate Medical Director for Harborside Hospice.

Dr. Tsai received his medical degree from the University of Hawaii John A. Burns School of Medicine in 1997. After graduating, he completed general internal medicine residency training at Lahey Clinic Medical Center in Burlington, Massachusetts followed by a geriatric medicine fellowship at Boston Medical Center.

In addition to his role as a hospice medical director, Dr. Tsai has extensive experience with the full spectrum of care settings after having worked as outpatient geriatric practice primary care physician, consultant physician for palliative care at acute care/skilled nursing facility/LATCH, hospitalist physician, and PACE physician.

Dr. Tsai’s professional interests include serious illness communication and symptoms management.

Dr. Tsai enjoys native flower gardening, sewing, and writing with fountain pens.
