Harborside Hospice is seeking a compassionate and skilled Hospice Social Worker to join out interdisciplinary care team. This role provides emotional, psychosocial, and practical support to patients and families facing life-limiting illness. Salary: $74,880- $83,200
Qualifications:
Must be Massachusetts licensed (LSW LCSW or LISCSW)
Must have 1 year experience working with geriatrics
Bachelor’s degree required. MSW degree desirable
Job Description: Provide psychosocial care to hospice Individuals and their families regarding economic, social, and emotional needs. Implement and coordinate all social service activities of the hospice program under the direction of the Hospice Medical Director.
Key Responsibilities:
· Conduct comprehensive psychosocial assessments for hospice patients and families
· Develop and implement individualized care plans in collaboration with the interdisciplinary team
· Provide counseling and emotional support related to illness, death, and bereavement
· Assist with advance care planning, including advance directive discussions
· Connect patients and families with community resources (financial assistance, housing, caregiver support etc.)
· Maintain accurate and timely clinical documentation in compliance with regulatory standards
· Provide bereavement follow-up services to families after patient death
