Job Title: On-Call Hospice Registered Nurse (RN)

Harborside Hospice is seeking a compassionate, experienced Registered Nurse to join our hospice team as an On-Call Hospice RN. The On-Call RN provides after-hours nursing care, triages patient concerns, responds to urgent visits, and supports patients and families with compassionate end-of-life care in homes, assisted living facilities and nursing facilities.

This role requires excellent clinical judgment, strong communication skills, and the ability to work independently while collaborating closely with the interdisciplinary hospice team.

Position location: Massachusetts Northshore/ Southern NH

Job Type: On Call

Hours Per Week: Saturday 8:30 a.m.- Monday 8:30 a.m.

Salary:

This role requires excellent clinical judgment, strong communication skills, and the ability to work independently while collaborating closely with the interdisciplinary hospice team.

Qualifications

• Current RN license in MA & NH
• Minimum of two years of nursing experience required.
• Hospice, long-term care or palliative care experience preferred.
• Valid driver's license, reliable transportation, and proof of automobile insurance.
• Strong assessment, communication, and critical-thinking skills.
• Ability to work independently and make sound clinical decisions in the field.
• Experience with electronic medical records. 

Responsibilities

• Respond to after-hours telephone calls from patients, families, caregivers, and referral sources.
• Perform urgent and emergency hospice visits as needed.
• Admit appropriate individuals to hospice care as requested during after-hour periods.
• Assess patients' physical, emotional, and psychosocial needs.
• Manage pain and symptom control according to hospice standards and physician orders.
• Provide patient and caregiver education regarding disease progression, medications, and comfort measures.
• Coordinate care with physicians, nurse practitioners, social workers, chaplains, hospice aides, and other members of the interdisciplinary team.
• Accurately complete electronic documentation in a timely manner.
• Maintain compliance with all federal, state, and hospice regulatory requirements.

Job Title: Hospice Social Worker (LSW, LCSW OR LICSW required)

Harborside Hospice is seeking a compassionate and skilled Hospice Social Worker to join out interdisciplinary care team. This role provides emotional, psychosocial, and practical support to patients and families facing life-limiting illness. The ideal candidate is empathetic, organized, and has a strong ability to navigate complex family dynamics and community resources. 

Position location: Massachusetts Northshore (Essex and Middlesex County)

Job Type: Full Time

Hours Per Week: 40.00

Salary: $74,880- $83,200

Qualifications:

Must be Massachusetts licensed (LSW LCSW or LISCSW)

Must have 1 year experience working with geriatrics

Bachelor’s degree required. MSW degree desirable 

Job Description: Provide psychosocial care to hospice Individuals and their families regarding economic, social, and emotional needs. Implement and coordinate all social service activities of the hospice program under the direction of the Hospice Medical Director.

Key Responsibilities:

• ·      Conduct comprehensive psychosocial assessments for hospice patients and families
• ·      Develop and implement individualized care plans in collaboration with the interdisciplinary team
• ·      Provide counseling and emotional support related to illness, death, and bereavement
• ·      Assist with advance care planning, including advance directive discussions
• ·      Connect patients and families with community resources (financial assistance, housing, caregiver support etc.)
• ·      Maintain accurate and timely clinical documentation in compliance with regulatory standards
• ·      Provide bereavement follow-up services to families after patient death
