Services
What We Offer
Harborside Hospice, LLC provides services that are available on a 24 hour basis to meet individual care needs of our patients. In addition to regularly scheduled visits, we provide additional visits as warranted for the palliation and management of conditions related to the terminal illness. All services are provided in a manner consistent with the accepted standards of practice.
Established in 2016, Harborside Hospice, LLC is committed to being here for you and your caregiver. We strive to provide support in all your care needs, whether physical, spiritual or social.
Routine Home Care
Hospice provides to the patient the full spectrum of hospice services, medications, medical equipment, and supplies that are related to the terminal illness. These services are provided in a variety of settings including, but not limited to, private residences, skilled nursing facilities and assisted living facilities. Hospice is reimbursed at a per Diem rate by Medicare, or private insurance.
Continuous Care
A change of status to Continuous Care may be provided when warranted during the periods of acute medical crisis for patients residing at a private residence or in a long term care facility. The patient must require nursing care for palliation or medical management of acute medical symptoms to be eligible to receive this benefit.
General In-Patient Care
If the clinical care needs of the patient increase to the point where care at home is no longer sufficient to effectively manage the patient’s pain and symptoms, then more direct clinical care by hospice staff can be provided in a setting outside the home (most often a hospice unit, skilled nursing facility, or hospital).
Respite Care
A short term benefit that provides the primary caregiver of a patient, who resides at a private residence, with up to 5 consecutive days of Respite Care on an occasional basis. This care is provided in a contracted skilled nursing facility and is reimbursed by Hospice at a per Diem rate for room & board and nursing care provided by the facility. Hospice continues all routine services as noted above.
